ELAINE ELAINE CARE BEYOND SURVIVAL Elaine connects Arizonas homeless and underserved e. g. low income uninsured seniors to social and community services vital to an individuals health and wellbeing. By providing empathetic and accessible transportation we reduce barriers to the Social Determinants of Health including Healthcare Medical appointments and prescription refills Food access Grocery store and food boxes Employment Interviews and day jobs Housing Interviewsand lease signings Criminal Justice Court and case management appointments Economic Opportunity DES and bank visits Educational Opportunity Parenting and financial literacy courses Social and Cultural Cohesion Community centers and support groups Critical errands Laundry mobile phone providers etc.
