TO PROVIDE HEALTH EDUCATION AND SOCIAL SERVICES TO INDIVIDUALS AND FAMILIES OF AFRICAN AMERICAN, HISPANIC AND CARIBBEAN DESCENT WHO LACK ACCESS TO ECONOMIC, SOCIAL AND HEALTH RESOURCES.
