We are a community-based program designed to service the needs of adults who suffer from Alzheimer's, Dementia, Progressive Memory Loss or Parkinson's. Ascend Foundation is a 501(c)(3) non-profit organization that supports the senior initiative to age in place. The Ascend Foundation provides residential, respite, transportation and nutritional supportive services to the elderly and frail and those functionally impaired older adults who desire not to be institutionalized.
