As an Adult Care Manager, you will provide outreach and enrollment services to adults who meet the eligibility requirements of New York State’s Adult Health Home program. You will help to organize client care to improve client health outcomes and reduce unnecessary emergency room and inpatient hospitalizations. Work Schedule is Monday through Friday, 8:00am to 4:00pm, with flexibility to meet client needs.
