MISSION IS TO DEVELOP, OWN AND MANAGE A PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY (PACE) IN CONFORMANCE WITH PACE LEGISLATION PURSUANT TO SECTION 1934 OF THE SOCIAL SECURITY ACT THAT PROVIDES COMPREHENSIVE MEDICAL AND SOCIAL SERVICES TO THE FRAIL, COMMUNITY DWELLING ELDERLY POPULATION OF THE GREATER ADIRONDACK REGION OF NEW YORK STATE.

The PACE at Hudson Headwaters team works with each participant to develop a care plan that promotes independence and the highest quality of life while allowing choice and dignity for participants and their families. PACE at Hudson Headwaters services select zip codes in Saratoga Warren and Washington counties. Most PACE participants are dualeligible meaning they qualify for both Medicaid and Medicare although there are also private pay options.
