Southern California Health & Rehabilitation Program (SCHARP) Southern California Health Rehabilitation Program SCHARP Mental health and social services Home Mental Health Matters Southern California Health Rehabilitation Program SCHARP was founded February 1994 as a nonprofit entity for the purpose of providing mental health and rehabilitation services to vulnerable ethnic minority populations in Southern California. Founded by two African American psychiatrists who were disillusioned with the traditional services provided to severely and persistently mentally ill individuals who are members of minority groups they developed services that meet the special needs of these communities along with those of the homeless mentally ill and homeless dually diagnosed. Learn More Programs A SchoolBased Program that provides comprehensive mental health services to children and youth in an effort to ameliorate symptoms of mental illness Services designed to reduce Mental Health Substance Abuse and Domestic Violence issues that create obstacles with a welfare recipients ability to work An integration of primary care and behavioral healthcare that is specifically targeted towards adults with serious mental illness SMI Supportive mental health services for unserved underserved or inappropriately served young adults who are unwilling or cannot access mental health services Empowering youth and their families to walk away stronger happier and healthier by utilizing their strengths strategies and support systems in their community Community reintegration established by California Law to reduce the state prison population for those individuals who meet qualifications Full Service Partnership FSP is a comprehensive and intensive mental health program for adults with severe and persistent mental illness The ICMS strives to improve the health outcomes reduce recidivism and create lasting social safety nets for vulnerable justiceinvolved individuals A patientcentered mental health program that utilizes evident base clinical practices shown to have good mental and physical health outcomes Provides client assessment housing navigation and community regional coordination for homeless and housing services for South East LA A comprehensive integrated evidencedbased treatment to the homeless severely mentally ill andor dually diagnosed population in South Central LA SCHARP has partnered with the Department of Mental Health to launch the Outreach Triage Team to serve Transitional Age Youth and Adults in South L.
