Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clar County.We work to coordinate services and support systems across a network of state and local organizations and help empower our community by providing access to information and support resources.At Sourcewise, we involve networks of state and local organizations together in a coordinated manner to provide adults with integrated access to service and support systems.
