Transition Services for High School Students - Trent's Team Trents Team High School Transitions We help high school students with disabilities transition from high school to adulthood. What We Do High School Transitions Our program provides transition services to high school students that have disabilities ages 1821. We teach our students necessary life skills including social skills and job training to communication skills and financial management. Supported Community Connector Providers assist with a variety of activities to participate in while in the community.
