CHDI’s mission is to continuously improve the Health and Well Being of the community we serve. This includes individuals without any form of health insurance, the underinsured, and individuals who receive public insurance. During Calendar Year 2020, we served approximately 10,400 individuals. The majority of those over 65 have multiple chronic conditions, such as Diabetes, Heart Disease, and High Blood Pressure. Also, 10% of our patients are Migrant/Seasonal Agriculture workers. We employ over 110 staff members from the community.
