VISION Our vision is to make medical and behavioral health services available to individuals in need and to reduce the instances of individuals affected by HIVAIDS. OUR HISTORY Outreach Community Care Network was established in 1985 as a nonprofit 501c3 organization in response to the Human Immunodeficiency Virus HIV epidemic. In 2020 we expanded our continuum of care services to include primary care substance use psychiatric and mental health services for all persons.
