The mission of Legacy Health Connections is to work collaboratively with other entities to provide resources, services, and educational programs to youth in and transitioning out of foster care, as well as other youth in under-resourced circumstances. Our aim is to improve their wellbeing, and the wellbeing of their relative and non-relative caregivers. And empower them to live and serve their communities with excellence.
