Arizona Chronic Care Together ACT is a coalition of patient health organizations representing the one in two Arizonans with chronic disease. We seek to shape policy decisions that build healthier communities. Member Organizations Positions taken by the coalition represent the majority of its members and do not necessarily reflect every members position on a particular issue.
