Through our collective impact model, we convene and collaborate with stakeholders across our community to address the root causes of homelessness and drive true systemic change. We embrace a data-driven and human approach - advocating for policies, incubating new ideas and programs, and investing in strategies to connect more of our homeless neighbors to a stable home and prevent homelessness before it begins. Our work is grounded in a commitment to advancing racial and socioeconomic justice - and working together, we can build a community where homelessness is rare, brief and non-recurring.

“All it takes is one catastrophe – whether it’s mold in the house or your car breaks down. There has to be more resources for people like me who do work,” says Linda Nguyen, a San Jose mom who was able to keep her and her 14-year-old son housed with help from the Santa Clara County Homelessness Prevention System. They are one of the 18,400 households – more than 33,000 people – who were able to stay stably housed through the program, which is proving that providing flexible financial assistance and supportive services is a successful and cost-effective intervention to homelessness.

As one of the many affordable housing developments funded by the 2016 Measure A housing bond, People Assisting the Homeless - PATH Villas on the Park is helping end and prevent homelessness by offering its residents: 🏠 84 units of permanent, supportive housing (that come fully furnished. ) ❤️ onsite case management services 🩺 medical services through Valley Homeless Healthcare Program's Hope Clinic 🐾 a Pet Pantry with Humane Society Silicon Valley 🎨 fun daily activities Everything residents need to thrive. 🏘️🌱.
