Since its inception, Hearts & Minds has provided services to over 6,000 individuals living with dementia and their families. Hearts & Minds Activity Center
✅Every year, we conduct an evaluation of our services and outcomes to understand our community impact. This last fiscal year, we served 134 program participants with dementia and 146 of their caregivers through 240 intergenerational activities, 197 hours of community entertainment, and 527 hours of support groups and trainings. 👐 These services are made possible by our wonderful families, community volunteers, and donors.
