Heart 2 Heart Collaborative improves the care experience for patients who have pediatric cardiac and congenital heart conditions and their families through education and advocacy. Founded in early 2014 by parents of children with heart defects H2HC partners with patients health care teams to positively impact their lifelong care received both inside and outside of the hospital. .
