We have worked for nearly 40 years to improve care for everyone. Last year nonprofit health plans in Minnesota Helped 0 Million people pay for care Paid Over 0 Billion in medical bills Paid Nearly 0 for medical care every second Spotlight Care coordination is a unique benefit which provides highly individualized holistic care to support members health and wellbeing. For patients with complex and chronic conditions care coordination enables personalized responsive care to enhance the patient experience reduce the cost of care and improve the health of communities throughout Minnesota.
