D/B/A PIVITAL PUBLIC HEALTH PARTNERSHIP (PIVITAL) IS A NOT-FOR-PROFIT ORGANIZATION LOCATED IN NEW YORK THAT OPERATES A RURAL HEALTH NETWORK AS DEFINED IN PUBLIC HEALTH LAW ARTICLE 29A WITHIN CHEMUNG, SCHUYLER, STEUBEN, YATES, ONTARIO, WAYNE, LIVINGSTON, AND SENECA COUNTIES. PIVITAL'S PURPOSE IS TO PROMOTE, INTEGRATE, AND EXPAND THE PROVISION OF ADMINISTRATIVE AND MANAGEMENT SERVICES AND HEALTH SERVICES AND HEALTH SERVICE DELIVERY AMONG PUBLIC HEALTH DIRECTORS IN THESE COUNTIES TO IMPROVE ACCESS TO CARE, HEALTH OUTCOMES, AND THE HEALTH STATUS FOR THEIR RESIDENTS.
