Transitions LifeCare was founded in 1979 as Hospice of Wake County by Dr. Billy Dunlap, a hematologist and oncologist in Raleigh, North Carolina, along with nurses from Rex and WakeMed hospitals. The first patient served was a young child with leukemia. In the subsequent years, the organization expanded its services and partnerships, launching its Palliative Care program in the 2000s. A significant milestone was the opening of the Reid S. Towler Campus in 2011, the first free-standing hospice facility in the area. In 2013, the organization changed its name to Transitions LifeCare to reflect its expanded services beyond traditional hospice care.

Impact

Transitions LifeCare operates in several counties, including Durham, Franklin, Harnett, Johnston, Orange, and Wake. The organization is recognized for its innovative approach and commitment to providing dignified end-of-life care, particularly to underserved communities. Approximately 8% of its patients are indigent, which is four times the national average.
