Transitions LifeCare
Overview
Transitions LifeCare is a nonprofit organization originally founded as Hospice of Wake County in 1979. It has evolved to provide a comprehensive range of care and support services for individuals and families facing serious illnesses or nearing the end of life.
Mission and Philosophy
The mission of Transitions LifeCare is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illnesses. The organization's philosophy emphasizes that every individual deserves the quality of life they desire during these challenging times.
Services
Transitions LifeCare offers a diverse array of services, including:
- Hospice Care: Providing comfort, peace, and hope to those with terminal illnesses.
- Palliative Care: Focused on alleviating symptoms and improving the quality of life for patients with serious illnesses.
- Transitional Home Care: Supporting patients during transitions from hospital to home.
- Bereavement and Grief Support: Offering counseling services for individuals and families dealing with loss.
- Education and Community Support: Providing resources and support for caregivers and the broader community.
History
Transitions LifeCare was founded in 1979 as Hospice of Wake County by Dr. Billy Dunlap, a hematologist and oncologist in Raleigh, North Carolina, along with nurses from Rex and WakeMed hospitals. The first patient served was a young child with leukemia. In the subsequent years, the organization expanded its services and partnerships, launching its Palliative Care program in the 2000s. A significant milestone was the opening of the Reid S. Towler Campus in 2011, the first free-standing hospice facility in the area. In 2013, the organization changed its name to Transitions LifeCare to reflect its expanded services beyond traditional hospice care.
Impact
Transitions LifeCare operates in several counties, including Durham, Franklin, Harnett, Johnston, Orange, and Wake. The organization is recognized for its innovative approach and commitment to providing dignified end-of-life care, particularly to underserved communities. Approximately 8% of its patients are indigent, which is four times the national average.
Awards and Recognition
Transitions LifeCare has been recognized for its quality care, earning designations such as a Hospice Honors recipient multiple times and being named one of the Best Places to Work.
Service Area
The organization serves residents across multiple counties in North Carolina, ensuring a broad reach of compassionate care services.
Financial Support and Partnerships
Transitions LifeCare depends on financial support from the community and partnerships to sustain its services, especially for those who cannot afford them. It remains an independent 501(c)3 organization, welcoming both financial and volunteer contributions to continue its mission.
