We continue to make strides towards a healthier future in our East End community by addressing one of the leading challenges of Richmond, Virginia's East End—hypertension. In 2024, we piloted the Hypertension Management Program, designed to give patients the tools they need to manage their high blood pressure beyond the hospital and doctor’s office walls. From nurses checking blood pressure numbers and teaching participants how to self-monitor their blood pressure, pharmacists offering medication management and working with primary care providers for any medication adjustments, registered dietitians teaching nutrition education and leading cooking classes for program participants, as well as community health workers conducting health needs assessments and providing support, the program involves several key health care team members.
