Opened in fall 2020 the CPHA offers integrated wholeperson care that addresses physical and behavioral needs as well as social determinants poverty housing transportation food insecurity etc. that impact a persons ability to both access care and live a healthy life. The CPHA will help people in southeastern Michigan to live longer and better through an integrated continuum of primary medical and behavioral care for adults and children with an emphasis on early intervention including screenings and preventive care.
