Family Rise Together - Strength In Numbers Family Rise Together Programs and Services Housing Multiple Programs FRT is a provider of Housing Stabilization Services a Minnesota Medical Assistance benefit to help seniors and people with disabilities including mental illness and substance use disorder find and keep housing. We leverage Family Homeless Prevention Assistance Program FHPAP funding and the Housing Trust Fund Rental Assistance Program HTF to helpachieve housing stability and transition topermanent sustainable housing opportunities. We also providesafe personcentered living arrangements and supplemental services to eligible individuals experiencing housing instability andor longterm homelessness LTH in communities of their choice using Housing Support dollars. Healthy Place Mental Health Wellness Services Adult Rehabilitative Mental Health Services ARMHS is a highimpact program that addresses the wellbeing of individuals with mental illness by providing culturally specific mental health services that emphasizes trust respect dignity and compassion.
