DBA ANGELES COMMUNITY HEALTH CENTER (THE "ORGANIZATION") IS A NON-PROFIT CORPORATION ESTABLISHED ON MAY 30, 2013 UNDER THE LAWS OF THE STATE OF CALIFORNIA.THE ORGANIZATION IS DEDICATED TO PROVIDING COMPREHENSIVE PRIMARY MEDICAL CARE, DENTAL SERVICES AND CARE COORDINATION, FOR THE CARE AND WELL-BEING OF ALL LOS ANGELES RESIDENTS. THE ORGANIZATION PROVIDES HIGH QUALITY, COMPREHENSIVE, PRIMARY AND PREVENTATIVE MEDICAL, DENTAL AND BEHAVIORAL HEALTH CARE SERVICES AND EDUCATION IN AN ENVIRONMENT OF CARING, RESPECT, AND DIGNITY. THE GOAL IS TO BE RESPONSIVE TO THE NEEDS OF THE COMMUNITIES THAT WE SERVE BUT WITH CONTINUED EMPHASIS ON THE UNDERSERVED AND THOSE WITHOUT ACCESS TO CARE REGARDLESS OF RACE, ECONOMIC STATUS, AGE, SEX, SEXUAL ORIENTATION, OR DISABILITY.THE ORGANIZATION OPENED A THIRD CLINIC IN GARDENA, CALIFORNIA IN APRIL 2019, AND ADDED DENTAL SERVICES TO THEIR HUNTINGTON PARK LOCATION IN SEPTEMBER 2019.
