Home - First Nations Community HealthSource Friendly Staff Welcome to First Nations Community HealthSource First Nations Community HealthSource based in Albuquerque New Mexico serves as New Mexicos Urban Indian Health Center and a Federally Qualified Health Center. With a legacy of over 50 years First Nations is deeply committed to providing an integrated and culturally sensitive health delivery system that addresses the physical social emotional and spiritual needs of the community members. The organization operates three clinic sites five schoolbased health centers and two outreach programs in Gallup and Farmington all dedicated to delivering comprehensive care. In 2019 the organization extended its services by opening an additional clinic site in Albuquerque with a focus on pediatric and womens health care services.
