HealthICT At HealthICT we collaborate with healthcare providers to develop innovative teambased care strategies focused on preventing and managing chronic conditions such as diabetes hypertension and high cholesterol. As an affiliate of the Medical Society of Sedgwick County we have been assisting providers in Wichita and across the state by providing free technical assistance since 2015 which has been made possible through a series of CDCfunded grants. In 2024 we were awarded funding through the Physicians Foundation to assist providers with identifying and assisting patients who face Social Determinants of Health challenges and how local referral platforms can help bridge the gap.
