Calumet Area Community Health Foundation Promoting Community Health and Education in Calumet County Communities Served by Ascension Calumet Hospital The Calumet Area Community Health Foundation was founded in 1998 to promote community health and health education in Calumet County Wisconsin through direct and indirect support of Ascension Calumet Hospital. The Foundation is administered by a local Board of Directors made up of women and men from this geographical area. Our Impact Since its founding in 1998 CACHF has awarded over 6. 1 million in grants to individuals and organizations in communities served by Calumet Ascension Hospital Brillion Chilton Hilbert Kiel New Holstein Potter and Stockbridge.
