The Program of All-Inclusive Care for the Elderly (PACE) model centers on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. Bluestem PACE provides care facilitated by an interdisciplinary team made up of a primary care physician, clinic and home care nurses, CNA’s, a physical therapist, occupational therapist, registered dietitian, day center manager, recreational therapist, social worker and transportation director. Our mission is to help people live safely at home long term by positive impacting quality of life through lifelong wellness, purpose, choice, genuine connections, and personalized care with Christian compassion.
