Before being discharged following a Stroke, a member of our Quality team will provide the patient a stroke education guide and discuss personal risk factors, medications they are and will be taking, and benefits of rehabilitation and additional follow-up care through our NRH Stroke Clinic. After the patient is discharged, we continue to check in on the patient for 30 days to ensure they feel supported and are recovering well.
