Healthy Communities of Clinton County Coalition | Home Healthy Communities of Clinton County Making the Healthy Choice the Easy Choice Our Mission The Healthy Communities of Clinton County Coalition was formed in 2006 as a partnership of healthcare providers service care providers educators and community leaders who were interested in addressing issues of health in the Clinton County community. Now over 15 years later the coalition is still committed to reduce the risk factors contributing to chronic disease including health disparities by decreasing stigma and encouraging policy system and environmental changes. Our Programs Youth RISE. Youth Leadership Council This program promotes the prevention of teen pregnancy and the efforts to reduce the number of teens with STIs.
