Community Health Provider Alliance
Overview
The Community Health Provider Alliance (CHPA) is a nonprofit organization dedicated to providing cost-effective, high-quality integrated primary care and preventive services to low-income working families and individuals in Colorado. Founded as a 501(c)(3) tax-exempt organization, CHPA is focused on improving the quality and cost of healthcare through a value-based care model.
Mission and Vision
- Mission: To improve the quality and cost of care for the people its members serve by leveraging the strength of community health centers and fostering innovative healthcare practices.
- Vision: To be an admired nonprofit leader in value-based care, focusing on individuals and communities.
Values
CHPA adheres to core values that include accountability, adaptability, community, and compassion. These values guide its approach to patient care, emphasizing empathy, kindness, and understanding.
Activities and Programs
CHPA collaborates with its member community health centers to implement best practices in primary care, guided by the principles of high-performing primary care. It works closely with clinical staff to monitor quality outcomes and adhere to evidence-based guidelines. The organization also develops alternative payment methodologies and engages in payment reform to reduce healthcare costs and enhance patient satisfaction.
CHPA fosters partnerships with various payers to negotiate value-based contracts, including primary care capitation, pay-for-performance programs, and shared savings contracts. These partnerships empower member health centers with resources to deliver high-quality care and improve long-term sustainability.
Additionally, CHPA supports its members through quarterly performance reviews, clinic-specific improvement activities, and the sharing of innovations and best practices. The organization also provides training and resources for activities such as Annual Wellness Visits (AWVs) and team-based care strategies.
Impact
By focusing on a patient-centered medical home care model, CHPA ensures that patients receive comprehensive care that addresses both physical and social determinants of health. This integrated approach includes services such as physical, behavioral, and dental care, along with programs for payer enrollment, care management, patient education, and patient navigation.
CHPA's commitment to data-driven feedback and performance reporting helps reduce health disparities and prioritize patient care. The organization plays a crucial role in promoting affordable, high-quality healthcare within communities.
