Safety has become “Kinder and Gentler.” Is this a good thing? Today’s safety culture looks a lot different than it did a few decades ago. The old “command and control” model—where fear of discipline was used to drive compliance—is giving way to something that many in the safety field would argue is more effective: trust. Human and Organizational Performance (HOP) recognizes that workers are fallible and that errors are often symptoms of deeper system issues—not individual negligence. Instead of punishing mistakes, we investigate the context. Pioneers like Conklin say, “You can either blame and punish, or you can learn and improve, but you CANNOT do both!” James Reason believed that over 90% of workplace safety errors are mistakes (slips, lapses), and not intentional violations. At the same time, Psychological Safety and Health principles are helping leaders foster environments where employees feel safe to speak up, report concerns, and participate in continuous improvement.
